Medical Clearance Process & Insurance

The following steps MUST be completed before being cleared to participate in practice or competition. 

1. 2026-2027 Online Athletic Clearance Process ONLINE CLEARANCE PROCESS  – 
2. 2026-2027 Release of Liability - Complete Before Tryouts– MUST BE COMPLETED BEFORE ANY TRYOUT. 
3. 2026-2027 Pre Participation Survey & Physical – PHYSICAL/WET SIGNATURES FROM DOCTORS ARE REQUIRED (NO STAMPS – NO EXCEPTIONS)
